The Social Worker will lead psychosocial support initiatives, ensuring that the adult residents receive compassionate, high-quality care that addresses their emotional and psychological needs. Permanent, Full-time in office Monday to Friday. Key Responsibilities Clinical Casework: Conduct comprehensive psychosocial assessments and manage ongoing casework to support the mental and social health of residents. Care Planning: Develop, implement, and routinely review individualised care and intervention plans to ensure personalised support. Therapeutic Facilitation: Lead therapeutic support groups and provide individual counselling or crisis intervention as required. Multidisciplinary Collaboration: Actively participate in multidisciplinary team meetings to coordinate holistic care and contribute to integrated care planning. Stakeholder Liaison: Facilitate essential communication between families, internal departments, and external specialised services. Regulatory Compliance: Engage with external bodies, including the Department of Social Development (DSD) and Department of Health (DOH), while maintaining strict adherence to professional ethical standards and legislative requirements. Administrative Excellence: Maintain precise records, reports, and statistical data in line with organisational policy.
