The Social Worker assists residents and their families in understanding their care needs and navigating the intricacies of care planning, decision-making and provision of community-based services. Role opportunity We have an exciting opportunity for a highly motivated, person‑oriented individual to join our organisation as a Social Worker. This is a full-time role spanning two facilities: Lefroy Care Centre (BullCreek) & Mosman Park Care Centre (Mosman Park), Monday - Friday. The role focuses on promoting the well‑being of clients/residents and their representatives, providing information and support tailored to complex care and social circumstances, and assisting families with understanding care needs, care planning, decision‑making and provision of community-based services. You will work with a multidisciplinary team to empower clients and families to achieve their goals effectively. Key Responsibilities Undertaking social work assessments, including psychosocial assessments for all assigned clients in consultation with other team members Liaising with clinical staff/pharmacy and informing them of residents’ planned discharge dates for transport arrangements Case management may include supporting clients with State Administrative Tribunal (SAT) and guardianship processes Planning and undertaking social work interventions with clients, in conjunction with their families and the multidisciplinary team Providing care coordination, counselling, advocacy, mediation, referrals and support for clients when transferring them to either their home environment or a Residential Care Facility Providing consultation and support to Nursing and Allied Health professionals regarding psychosocial aspects of care for clients Ensure compliance is maintained in line with Amana Living’s policies and procedures, and accreditation standards are sustained Provide case management and discharge planning services to ensure the client can either remain with the provider as a permanent residential client, be discharged home (community) with appropriate supports, or be discharged to another provider What You’ll Have The successful candidate will demonstrate experience working in a fast-paced role in an organised and adaptable manner. Strong interpersonal skills are essential as this role will involve interacting with residents and their families and engaging the multidisciplinary team to provide an autonomous high level of care to meet the needs of our residents and their families. Essential Criteria Bachelor of Social Work Knowledge of the Aged Care Sector Strong time management skills and ability to prioritise tasks efficiently Excellent problem-solving and negotiations skills High attention to detail Exceptional verbal, written and interpersonal communication skills Evidence of Current Flu vaccinations per Amana Living policies Ability to obtain a National Police Clearance (within 6 months validity) or NDIS Screening check Successful completion of pre-employment medical and reference checks Why Choose Amana Living?
