What you’ll do Match caregivers to patients based on needs and availability Handle high-volume calls with professionalism and speed Keep patients, families, and caregivers informed and aligned Manage call‑offs, schedule gaps, and last‑minute changes Maintain accurate records in HHA Exchange Track updates through the Call Dashboard and support pre‑billing Process referrals and service inquiries Ensure care aligns with each patient’s Plan of Care Support, guide, and coordinate with care staff Flag issues and changes quickly to leadership What makes you a strong fit Experience in scheduling, coordination, or healthcare admin Strong attention to detail, you don’t let things slip Clear communicator (written + verbal) Comfortable juggling multiple priorities at once Calm under pressure in high‑call environments Skilled in Microsoft Office and scheduling systems Experience with HHA Exchange is a plus Spanish fluency is a plus Why this role matters You’re not “just scheduling.” You’re making sure patients receive the care they depend on, on time, every time.
