The RN Case Manager develops a patient centered, comprehensive plan of care utilizing a multidisciplinary team approach. The RN Case Manager is accountable for achieving positive patient outcomes Supervises Home Health Aides and LPN’s according to policy Reports daily to the Clinical Manager all pertinent patient information Reports/records information accurately Uses appropriate safety and infection control measures Home Care RN QualificationsGraduate of an accredited school of nursing with a current North Carolina RN license or RN multi-state licensure privilege through the Nurse Licensure Compact.
